Accreditation for Visiting Medical Officers at The Sydney Private Hospital

The Sydney Private Hospital welcomes Visiting Medical Officers (VMOs) to apply for accreditation to our facility.

Step 1: Please download the following form: Sydney Private Hospital Visiting Practitioner Rights Application then upload to our online Specialist Accreditation Application portal below.

Step 2: Please prepare the following documentation for upload

  • Current Curriculum Vitae
  • Photocopy of current AHPRA registration
  • Two written references
  • Hand Hygiene Certificate
  • Working with Children Check
  • Photocopy of current professional indemnity insurance certificate of currency showing:
    – Period of Insurance
    – Amount of Cover
    – Category of Insurance
    – Copy of Radiation License – if you intend to operate x-ray equipment.

Step 3: Complete the Specialist Accreditation Online Form
